IMPORTANT NOTICE ABOUT GHI

Our Valued GHI Insured Patients:

Effective January 1st, 2009, Dr. Marc D. Danziger and Dr. John Won will no longer be contracted with GHI. We regret taking this action but have been forced to do so as GHI reimbursement rates are below our costs of providing care.

As such you will be asked to pay for the all fees incurred in your visits at the time of service. We advise you to verify your GHI policy’s OUT OF NETWORK benefits. This will help you plan for future services.

Financial and Insurance Information

Our patients’ healthcare is our main concern.  Our doctors participate in many insurance plans. It is important for our patients to understand and follow their own insurance company’s requirements and to take advantage of the highest level of benefits in their policies. In this way our patients can reduce their out of pocket expenses. We will do our best to inform our patients of their insurance requirements which we are currently aware of.  However, requirements change often and it is ultimately the patient's responsibility to educate him/herself regarding their own policies and how they are implemented.

At the time of your visit, you will be asked to provide your insurance card for verification. If you forget to bring your insurance card you will be asked to provide a credit card for processing until if and when your insurance can be verified.  If you need a referral for your visit, please ensure that the referral is at our office prior to your visit.

For plans in which we do not participate, like MEDICAID, EMPIRE PLAN, etc., the patient is responsible for all fees for services rendered. Payment is expected at the time of service by credit card, check or cash. We will then process any insurance claims you may have for your direct reimbursement if your coverage provides out of network benefits.
