| Medicare Part B | does not require referrals, but you must disclose who is your referring doctor. Medicare patients that are being cared for at a Nursing Home (Skilled
Nursing Facility) need to advise our staff. This will help provide services
that are in accord with your healthcare status at the time of service. |

Normally does not require referrals.

| HIP (excluding Medicaid policies) |
requires paper referrals. There are a few new policy
types that do not need referrals. Note that you should advice our staff
of who your referring doctor or center is before we can setup your appointment. |
| Oxford Fredom Plans, HMO and Liberty Plan (Dr Reckler is not participant) |
HMO policies require referrals, Freedom policies
use referrals if the patient does not want to be liable for deductibles
and coinsurance. Freedom Select does not need referrals |
| Oxford Medicare Advantage |
always needs referrals since it has no out of network
benefits

| United Health Care (Excluding Medicare Complete and Medicaid policies) |
HMO requires the patient to have a United Health
Care PCP assigned. This PCP information needs to be disclosed to our staff
at the time of service. |

Choice PPO, this could change depending on 32 B-J
